CARE-PLANS
1st Choice Home Services understands that not one care plan fits all. Our team will take the time to get to know you and develop an individualized care plan that fits your specific needs.
Daily services can include but are nor limited to-anything from meal preparation, hygiene, cleaning, outings, supervision and more.
Consumer Directed Services
1st Choice Home Health Services goal is to make sure that all of our clients feel as safe as possible. With this service, you are able to choose your own worker- which includes a family member/loved one. This program essentially allows you to be your own boss
In-Home Services
Physical disabilities, Dementia and Age related conditions are some of the few circumstances that prevent patients from caring for themselves. Our team of caregivers can assist you with daily tasks such as mobility, eating, exercising, and grooming.
HEALTHY CHILDREN AND YOUTH (HYC)
The Healthy Children and Youth Program provides services coordination and authorization for medically necessary services for MO Health Net recipients with special health care needs from birth to the age of 21. This services includes In-home personal care and or skilled nursing visits.
